Under the supervision of the Director of Medical Management, this position is responsible for validating, integrating and coordinating the P3 inpatient list for all local acute and post-acute facilities. Through electronic interfaces, telephone calls and other sources of admission notification, discharge and transfer, this employee is responsible for an accurate patient census for all P3 admissions. He / she coordinates with assigned hospitalist for each facility. Appropriate patient identification enables P3 to enhance the achievement of the Quadruple Aim: improved outcomes, improved experience of care for patients and providers and lower healthcare costs.
Promote the mission, vision and values of P3 Health Partners
Coordinates patient census between UM, CM, hospitalists, facilities, internal stakeholders
Provides administrative functions to support IP functions and hospitalist team
Assists with interventions associated with managing ER utilization report and interventions, per policy, to address inappropriate ER utilization
Responsible for timely and accurate documentation in systems
May assist the UM / CM team with
Distributing census by required time
Posts hospital records to appropriate site
Admission / discharge notification to PCPs
Collect DC summary and distribute / post for PCP
Ordering HHC / DME / transportation, etc
Develops spreadsheets and other tools to support Medical Management team
Liaison between inpatient and outpatient team
Schedules and manages daily / weekly / inpatient rounds calls
Responsible for coordinating inpatient coverage calendar with hospitalists and posting schedules
Responsible for reporting any accounts payable shifts
Assigned to ad hoc outreach initiatives
Experience in a healthcare company; experience in a managed care organization preferred
Certified medical assistant preferred
Knowledge of medical terminology required
Excellent communication skills with patients, providers, internal and external customers required
Must have excellent computer skills
Must have excellent organizational skills and ability to work independently
High School graduate required
Education as a medical assistant or nursing school student
