Under the supervision of the Director of Medical Management, this position is responsible for coordinating the documentation and required activities for patients enrolled in a Special Needs Plan. Through enrollment data, communication with brokers and the Health Plan[s], SNP patients will be identified and processes managed to ensure confirmation of SNP eligibility and timely appointments with providers and the Care Team. Patients will be guided to enhance the achievement of the Quadruple Aim: improved outcomes, improved experience of care for patients and providers and lower healthcare costs.
Promote the mission, vision and values of P3 Health Partners
Coordinates SNP patient activities between the Health Plan[s], practices and Care Management
Provides administrative functions to support all SNP required activities and documentation
Makes telephone calls to patients to obtain information and make appointments.
Provides outreach and guidance to patients
Supports the Medical Management team with appropriate referrals, claims history or any other clinical information necessary
Responsible for timely and accurate documentation in systems
Develops spreadsheets and other tools to support SNP documentation
Becomes the Subject Matter Expert for SNP program requirements
Trains staff, Medical Group and Affiliates, regarding SNP program requirements
May assist patients in obtaining / coordinating community and other resources
Addresses gaps in care
Assigned to ad hoc outreach initiatives
Strong problem solving skills required
Certified medical assistant preferred
Experience in a managed care organization preferred
Knowledge of medical terminology required
Excellent communication skills with patients, providers, internal and external customers required
Must have excellent computer skills
Must have excellent organizational skills and ability to work independently
High School graduate required
Education as a medical assistant or nursing school student may be required
Performance Metrics [volumes will be entered depending on SNP membership]:
All SNP patient’s initial attestations signed and returned to Health Plan[s] within 30 days of effective date
All SNP patient’s have initial appointment with PCP within the first 30 days of effective date
In conjunction with the Care Manager, ensures the annual HRA is completed prior to 1 year from the previous one
