The Social Worker will collaborate with P3 providers, team members and patients to improve the lives of people who are in need of support and help. He / she will evaluate patients, determine their needs and put together an action plan to make sure that those needs are being met efficiently. The goal of this position is to enhance and maintain the patients’ physical, psychological and social function and to ensure the patients’ welfare is protected.
Collaborate, communicate and coordinate appropriate levels of care with hospitals, specialists, emergency department staff, patients, families and the Medical Management team
Provide social work knowledge and act as a P3 resource to the teams
Enter and maintain pertinent clinical information in various Medical Management systems
Works to manage health care costs and maintain high quality social service follow-up
Reviews Risk Stratification Tool and other reports to screen patients for social service intervention
Monitors pharmacy utilization and works with patients that may need assistance with medication costs
Assists patients and families to complete Medicaid applications
Coordinates guardianship paperwork expeditiously as needed
Address Advance Directives with patients; assists them as needed to complete
Participates in Interdisciplinary Team Rounds and Conferences
May visit with patients at home or in acute or post-acute facilities
Receives referrals from patients, families and P3 stakeholders
Address gaps in care
Documents real time in the P3 Medical Management and other systems
Experience and Qualifications:
Minimum four years of experience, including a minimum of two years in a managed care/HMO organization. Case management and utilization management experience helpful. Proficient with computer and software programs (e.g.; Microsoft Word, Excel) and the Internet required. Must be a self-starter, comfortable collaborating with physicians, hospital team members, patients and families; discussing appropriate levels of care that may be in conflict with requested service.
License: LCSW or MSW
Graduate from an Accredited Social Worker program
